I was struggling to close down a test program which was choking on the amount of media it was loading. When I finally got control back I saw this:

It's not meant to happen, you only meant to be allowed one copy open. May have bugged out as your computer was responding slowly and windows may not have noticed it was already opening task manager many times before.

I think I've managed to do this before. Never occurred to me that it wasn't supposed to happen, I just figured it was my fault for spamming the key combo.

I guess something like that was probably the cause.
