You'll need to be more precise about what you're doing. There are many, many different kinds of certification. But in the main, you sit down at a computer whenever you're ready and log on. They aren't scheduled, they are available all the time.

Difficulty is subjective. If you've done the course, they are straight forward if you're of average intelligence. If you go to a more suspect establishment with the sole objective of walking out with a piece of paper, then you may as well just stay in bed and ask them to post it.
