Great Idea, reminds me of the Arcade game called Chiller, do a search for arcade and Chiller, gruesume stuff. In fact a great idea would be a human model you could shoot up and bits fly off and show you whats under it, then you keep shooting and it gets more and more bloody and wrecked, just go to youtube and search the arcade game Chiller and get some great ideas.

Mad people in straight jackets is also great, remember those British Hammer House Of Horror films, scenes from the lunatic asylum are great, mad people are scary period. I remember Quake 2 had those shell shocked soldiers ranting in their cells, dead simple, but it freaked me out a bit, yep, nutters are great for scares.
