Make sure your model has not bad surfaces, double side surfaces, surfaces are fully triangulated.

Sorry no msn!

By the way I see from the picture you didn't triangulate the surfaces.

Make this steps.

Make a backup copy of your model

Then with the original model
If not a single object select all objects then menu Object -> Merge

Select the object, and Optimize vertices (Ctrl+Shift+V)

Select the object and menu Object -> Optimize surfaces

Select all the surfaces and click the 1S button in the bottom left
Set Surface Type. This way all your surface are 1 side only; if anyone disappears that it means the surface was 2 sided and now is 1 sided with no correct normal, you had to flip it (if it was a 2 sided surface and disappear it still exist but you can't view it from certain position)

Select all the surfaces and menu Surface -> Triangulate

Export as x meshes

Hope that helps

EDITED: having a closer look at your model it seems it's not well centered too

it would be better to make this a segment not an entity. FPSC entity collision is horrible.

TGC's segment maker can be found on the files section of fpscreator.com.
