the sound appears distorted and creates problems

This should work:

desc = sound repeat on injure

:state=0:state=1
:state=1,plrhealthgreater=22:stopsound
:state=1,plrhealthless=22,plrhealthgreater=1:loopsound=audiobank\music\Rafleo\Heartbeat.ogg,state=2
:state=2,plrhealthgreater=22:stopsound,state=1

I didn't test it so it may not work.
