Okay, basically all I need to get Room 101 going is get a HTML form that does the following:

1. Get the name of the person who made the project
2. Get the name of the project
3. Three radio buttons with these titles:
1. Project is complete
2. Project is near completion
3. Project is midway
4. A browsing directory that lets you look for the source file
5. Email this information and source code to an address

If anyone could do this for me I would be extremely grateful.
