Do a search on level design on this forum and you'll find what you're looking for. For starters, you could check out http://www.delgine.com and download DeleD, my free 3D editor in the making, suitable for level design.

3D State has a world builder that is free I believe. At least it came free with a recent issue of 3D World Magazine. I don't know how easy it is to import in to DB... but hey, it's free. What do you lose from trying it out.
